ACUITY is a patient-centered care management software system that performs integrated utilization, case and disease management.
After a comprehensive evaluation and selection process, PANS chose TCS Healthcare Technologies over other care management software vendors for several reasons. “The ability to customize and be flexible were absolutely essential for us, especially because this is a new program and we will need to make changes as we go. We were a start-up company for care coordination, and knew that our program, and consequently our infrastructure, would be evolving,” says Kathleen Miodonski, RN, BSN, CMAC, vice president of clinical operations. “We chose TCS because they could handle the complexities of our reporting requirements. The TCS team really listened and understood our needs.”

Post-Acute Network Solutions also contracts with select Medicare-Medicaid Alignment Initiative (MMAI) Health Plans to provide care coordination services for higher risk members who are eligible for long-term services and support (LTSS) or home and community-based (HCBS) waiver services. Post-Acute Network Solutions’ Network connect offers partnering health plans and their dual-eligible beneficiaries an in-person CareCoach, delegated utilization management and coordination with an integrated care team. Currently, only 25 other states are participating in this initiative, giving PANS a unique advantage over competition.
