White Plains, NY, May 14, 2013—Alicare Medical Management (AMM) received the prestigious Case in Point Platinum Award for its diabetic management program during Dorland Health’s 5th Annual Care Coordination Summit in Washington, DC. Case in Point Platinum Awards recognize professionals and organizations making significant headway to address the challenges impacting the U.S. health care system.

As part of its “Getting Well, Staying Well” initiative, AMM developed and implemented the diabetic management program in conjunction with a trust fund client that covered a population suffering from a high rate of diabetes. The program allows the waiving of all co-pays, not just for diabetes medication, as long as the patient sets and achieves goals with the case manager. Thanks to strong incentives, the client achieved a 60 percent enrollment rate of identified diabetics within the first year of the program and saw a 95 percent retention rate. As a result, 100 percent of enrolled clients met blood glucose monitoring compliance.

According to the Centers for Disease Control and Prevention (CDC), diabetes impacts 25.8 million people, more than 8 percent of the entire U.S. population, and is the seventh leading cause of death in the United States. It is a major contributor to heart disease, stroke, kidney failure, limb amputations and blindness. The CDC estimates medical expenses for diabetics to be 2.3 times higher than medical expenses for those without the disease, and costs our nation $116 billion in direct medical expenses a year.
