TCS Software to Support Care Coordination Plans for Senior Residential Facilities and Medical Providers

Auburn, CA / PRWeb / September 23,  2014 – Post-Acute Network Solutions (PANS), a company that specializes in care coordination models for managed care plans, senior residential facilities and medical providers, has successfully implemented TCS Healthcare Technologies’ ACUITY Complete Care™ (ACUITY) care management cloud hosted system to support their managed care organization model.  ACUITY is a patient-centered care management software system that performs integrated utilization, case and disease management.

After a comprehensive evaluation and selection process, PANS chose TCS Healthcare Technologies over other care management software vendors for several reasons.  “The ability to customize and be flexible were absolutely essential for us, especially because this is a new program and we will need to make changes as we go. We were a start-up company for care coordination, and knew that our program, and consequently our infrastructure, would be evolving,” says Kathleen Miodonski, RN, BSN, CMAC, vice president of clinical operations. “We chose TCS because they could handle the complexities of our reporting requirements. The TCS team really listened and understood our needs.”

Craig Reiff, principal, added, “The ability to do the software in a hosted environment was key as well. TCS also has an incredibly long list of tracking metrics; it was one of the few software programs that could back the wide-ranging requirements of PANS. The implementation speed was really done well, and the TCS team met all the key timeframes.”

Deborah Keller, RN, BSN, director of client services for TCS, says the implementation team is comprised of members with technical and managed care backgrounds. “We understand both ends of the spectrum and work with our clients every step of the way,” she says. “Having both the clinical and technical expertise really differentiates TCS from competitors.”

According to TCS founder and CEO Rob Pock, the TCS clinical and technical staff worked closely with the PANS team to ensure the timeline and technical criteria were addressed. “We have a passion for care management,” he says, “which means working with our clients throughout the implementation process to provide training, configuration and interfaces that ensure the technology, processes and clinical content meet their needs.”

“The TCS implementation team was more than willing to work with us on an extremely condensed timeline,” Miodonski continues. “We also really liked TCS’s affiliation with the Case Management Society of America (CMSA). In fact, CMSA tools are incorporated into the ACUITY system.”

Post-Acute Network Solutions also contracts with select Medicare-Medicaid Alignment Initiative (MMAI) Health Plans to provide care coordination services for higher risk members who are eligible for long-term services and support (LTSS) or home and community-based (HCBS) waiver services. Post-Acute Network Solutions’ Network connect offers partnering health plans and their dual-eligible beneficiaries an in-person CareCoach, delegated utilization management and coordination with an integrated care team. Currently, only 25 other states are participating in this initiative, giving PANS a unique advantage over competition.
